Procedure: Closed revision rhinoplasty in men after two previous open approaches, correction of structural deformities, and removal of a problematic graft.

Goal: Restore nasal function, protect skin integrity, and achieve a natural masculine appearance following unsuccessful prior surgeries.

This patient had undergone two previous rhinoplasties, both through an open approach. These procedures left him with significant anatomical distortion of the nasal tip, leading to both aesthetic deformity and functional compromise due to nasal pinching, a condition that narrows the airway and obstructs breathing.

In this case of revision rhinoplasty in men the greatest concern, however, was localized at the tip skin. A previously placed oversized Sheen graft, combined with the patient’s naturally thin skin, was progressively damaging the overlying tissue. The pressure exerted by the graft edges threatened to erode through the skin, jeopardizing both appearance and function.

Dr. Escobar performed a closed revision rhinoplasty in men using his Modular Technique, an evolution of the Combined Auricular Graft method he has published. This revision rhinoplasty in men, included careful removal of the problematic graft, reconstruction of the tip framework with auricular cartilage, and the use of the patient’s own fibro-fatty tissue to reinforce and protect the delicate nasal tip skin. This approach not only restored structural support and improved airway function but also optimized healing by preserving blood supply and preventing further thinning of the tip skin.

  • Elimination of the visible graft under the nasal tip and protection of the delicate tip skin.
  • Correction of nasal pinching and improved breathing function.
  • Restoration of a more natural and masculine nasal contour.
  • Enhanced nasal symmetry and refinement of tip definition.
  • Long-term stability through reinforced structural support

💡 Key Consideration in Revision Cases: In revision rhinoplasty, the primary goal is to achieve both aesthetic improvement and functional restoration. Since patients have already undergone previous procedures, the pursuit of absolute perfection is neither realistic nor advisable. Instead, each case is carefully evaluated to define what can be safely improved, with all limitations and expectations discussed transparently during the preoperative consultation.

Closed Revision Rhinoplasty

Before my surgery with Dr. Escobar, I had undergone two previous procedures that left me with an unnatural appearance and ongoing breathing problems. <br><br>The last surgery became complicated, as a graft was placed in the nasal tip which, over time, became increasingly noticeable.<br><br>From my very first consultation with Dr. Escobar, I felt the confidence I needed to undergo surgery for the third time. My recovery was smooth and without complications, and within just a few days I was already feeling well.<br>Throughout the entire process, Dr. Escobar was attentive to my progress. <br>After my surgery with him, my nose looks much more natural and, most importantly, I can breathe freely again.

For women and men—whether primary or revision—my intent is to achieve natural, balanced results that blend seamlessly with the face while preserving—often improving—breathing. Guided by facial proportions (Golden Ratio), I design a tailored surgical plan for each patient. When appropriate, I favor the endonasal (closed) approach to refine with precision, reinforce structure, and avoid visible external scars, aiming for long-term stability and function. Every case receives my direct oversight; we set realistic goals together, so results enhance beauty, respect individuality, and build confidence.

Revision rhinoplasty. Secondary surgery is both technically complex and emotionally demanding. I carefully review your prior history, support you throughout the process, prioritize restoring function, correct asymmetries, and pursue a soft, natural look.

About these galleries. The cases shown (women and men; primary and revision) are published with explicit patient consent. They are educational, illustrating the kind of natural and functional outcomes that may be achievable in similar situations. Final results vary according to each patient’s anatomy, previous procedures, and individual healing.
