Procedure: Closed (scarless) wide nose rhinoplasty with septal and auricular cartilage grafts

Goal: Strengthen weak nasal tip support, refine the wide base, lengthen the septum, and improve overall nasal definition and profile balance.

This male patient presented with a wide nasal base, weak tip support, and short septum, typical features of an ethnic nose. A closed wide nose rhinoplasty was performed using a structural approach:

  • Auricular conchal cartilage was harvested and shaped into a “seagull-wing graft” to reinforce the weak alar cartilages and provide tip strength and definition.
  • Septal cartilage grafts were used to lengthen the septum, improving nasal projection and tip position.
  • The wide nasal base was reduced with precise alar base reduction techniques, placing incisions discreetly in the alar crease to minimize visible scarring.
  • Osteotomies (hinge technique) were performed through an endonasal approach, narrowing the bony vault without external scars.

The result was a more refined, elongated, and balanced nose, with a natural appearance that respects the patient’s ethnic identity while significantly improving both aesthetics and nasal function.
