Procedure: Male Revision Rhinoplasty (Endonasal Approach with Combined Auricular Graft)

Goal: Rebuild nasal support and function in a patient with thick skin and two prior surgeries, achieving a natural and balanced outcome.

This patient presented after two previous open rhinoplasties that had compromised the nasal tip support and left significant structural weakness. The thick skin added complexity, as it required careful planning to ensure that the nasal tip structure would adapt smoothly during healing, avoiding scar-related distortions of the tip and nostril contour.

A combined auricular cartilage graft was used in this Male Revision rhinoplasty to reinforce the nasal framework, restore projection, and improve tip definition. This Male Revision Rhinoplasty using the combined auricular graft was able to correct alar retraction and nasal pinching, while also enhancing airway function. The challenge was to provide stable long-term support strong enough to withstand the pressure of thick skin, ensuring that the final contour remained natural and symmetrical.
